HSE Questionnaire

In order to improve our practice in the fields of occupational health & safety, environmental protection and fire protection, and in accordance with our commitment to being a socially responsible company, starting from April 1, 2013, only service providers* (and their subcontractors) who meet the requirements of the legislation of the Republic of Serbia in the field of OSH, EP and FP will be able to participate in tenders and procurement procedures organized and conducted by NIS j.s.c. Novi Sad.

Exemptions from the process of HSE qualification

All Service Providers are required to go through the HSE Qualification Process, except in cases when they solely provide the following services:

  •          •  Services explicitly performed outside the NIS j.s.c. Novi Sad sites (servicing, gauging, calibration, etc.);
  •          •  Administrative services (translation, consulting, legal, software, engineering, designing, freight forwarding, logistics (without transport services), printing, banking, property valuation/registration - cadastral recording of buildings, marketing services not including working at height, etc.);
  •          •  Health services;
  •          •  Supply of the Company by suppliers and contractors, who are treated as third parties.

Partial exemption from the process of HSE qualification

For Contractors, providers of certain services, a partial implementation of the requirements of the Contractor Management Process is envisaged, as follows:

  •          •  For the servicing of equipment under warranty period and installed by them, solely the requirements of Process III, shall apply, namely the following activities: 3.2, 3.4, 3.5, 3.6, 3.7, 3.8, 3.9, 3.10.
  •          •  Services provided by public enterprises, institutions or authorities, as well as companies on the Non-Alternative Supplier List, are not subject to the obligation of HSE retraining/qualification at the Company's locations, but all other process requirements shall apply.

One of the basic prerequisites for continued business cooperation, with constant compliance with the legislation in the field of OSH, EP and FP, is the need for your company to be qualified in terms of occupational safety and health, environmental protection and fire protection, which is achieved through the HSE qualification process, i.e. completing the HSE questionnaire and submitting certain documents.

Important notes for completing and submitting the HSE Questionnaire

Interested Contractors are required to submit the HSE Qualification Questionnaire, completed by the appointed Occupational Safety and Health Person, signed by the Contractor responsible person, as well as all supporting HSE documentation.

All Contractors complete the HSE Qualification Questionnaire document in the manner described in the document itself.

  •     •  If the Contractor performs servicing (which is not in the field of environmental protection and in the field of accommodation and food for employees) then they should at least provide evidence to confirm fulfilment of the following:
  •         •  basic elimination criteria (Defined in Table 1, documents HSE Qualification Questionnaire_general, part 1 (Questionnaire)) and
  •         •  depending on the level of HSE service risk for which they apply, elimination criteria for moderate and high risk Contractors (Defined in Table 2, documents HSE Qualification Questionnaire_general, part 1 (Questionnaire)).
  •     •  If the Contractor performs activities in the field of environmental protection, they complete the HSE Qualification Questionnaire_EP the method of completing the questionnaire is the same as for other services, but they shall fulfil at least the following:
  •         •  basic elimination criteria (Defined in Table 1, documents HSE Qualification Questionnaire_EP, part 1 (Questionnaire)) and
  •         •  elimination criteria for work in the field of environmental protection (Defined in Table 2, documents HSE Qualification Questionnaire_EP, part 1 (Questionnaire)).
  •     •  If the Contractor performs activities in the field of accommodation and food for employees, they shall at least meet the mandatory criteria for work in the field of accommodation and food for employees (Defined in the document HSE qualification questionnaire_AFE)

The Contractor may also provide evidence for all other criteria listed in the HSE Qualification Questionnaire, as this way they will obtain a higher overall score, as can be seen at the bottom of the document. We would like to remind you that this Qualification Questionnaire is an indispensable part in the final selection of the Contractor.

Please submit the documentation (marked in accordance with the request to which it relates) confirming the statements from the Questionnaire, together with the completed HSE questionnaire, as well as updated and verified OSH forms (records) exclusively by e-mail to: hse.kvalifikacija@nis.eu - so that we can confirm the statements in the questionnaire and thus begin i.e. continue successful cooperation.

The requested documentation shall be submitted exclusively in electronic form, in Serbian or English.

All documentation submitted for the HSE qualification shall be zipped and sent with safety measures only by using mutually acceptable encryption methods, combined with appropriate procedures that together ensure the confidentiality of the data. Unless the submitted documentation is encrypted (password protected), the documentation will not be reviewed.
